Unregulated doctors' fees and patient choice versus uncontrollable insurance costs. Insurers try to introduce 'managed care' to control costs, doctors push back because it restricts their capacity to treat. The answer? Reduce the power of doctors and insurers in health governance, and make the test the public good. No more private health insurance, regulated fees, no more private hospitals that can cherry-pick patients, much more powerful community health options, evidence-based procedures etc.
